The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies.

• Coordinates total care plans with healthcare professionals and helps maintain continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.
• Conducts initial and comprehensive nursing evaluations, determines primary care focus, develops the care plan according to state guidelines, and submits accurate documentation in collaboration with physicians.
• Performs patient assessments at designated times and develops appropriate care plans.
• Ensures patients meet home health eligibility and medical necessity guidelines.

License Requirements:

• Current RN licensure in the practicing state
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation or public transit
