Job summary

Proforma Health is seeking a compassionate Home Health Registered Nurse to provide one-on-one care in patients' homes. You will develop and implement individualized care plans, administer medications, perform wound care, and conduct assessments.

The ideal candidate holds a nursing diploma or higher, with an RN license, CPR certification, and a valid driver’s license. Experience in med-surg, ICU, ER, or acute care is required; home health exposure is preferred.

You will develop and implement individualized care plans, administer medications, perform wound care, and conduct assessments. Your role includes monitoring patient conditions, educating families, maintaining accurate records, and collaborating with healthcare professionals.

Key responsibilities

supervising Home Health Aides, reporting patient progress, and ensuring quality care delivery.

The ideal candidate has a nursing diploma, Associate or Bachelor’s degree, and current RN licensure. Experience in med-surg, ICU, ER, or acute care is required, along with CPR certification and a valid driver’s license with reliable transportation. Home health experience and one year of nursing are preferred.

This role requires availability on weekends and two weekdays, with compensation ranging from $63.00 to $88.00 per visit, and annual salary estimates between $92,600 and $127,400, depending on location and experience.
