Job summary

Affinity Home Health Care Inc. in Pennsylvania seeks a Registered Nurse to provide and coordinate nursing care in patients' homes. You will perform comprehensive assessments, develop care plans with physicians, and supervise aides and LPNs/LVNs to ensure high-quality care.

You will document care, communicate with families and staff, and participate in orientation, safety initiatives, and on-call/weekend rotations. A PA RN license and reliable transport are required; CPR is preferred.

Qualifications

• Current RN licensure in the practicing state.
• CPR certification preferred or ability to obtain within 90 days.
• 1+ years of clinical experience preferred.

Responsibilities

• Develop and update patient care plans collaboratively with physicians and the healthcare team.
• Perform comprehensive nursing assessments, including OASIS and medication reconciliation.
• Provide or supervise clinical care and coordinate with aides and LPNs/LVNs.
• Document patient care accurately and communicate with physicians, patients, families, and staff.
• Participate in staff orientation, quality improvement, safety initiatives, and efforts to reduce hospitalizations.
• Maintain professionalism, punctuality, ongoing learning, and participate in on-call/weekend rotations.
• Adapt to diverse home environments and client backgrounds.

Qualifications & Skills
