Job summary

Cherokee Nation Home Health Services is seeking a compassionate Home Health RN to assess patients in their homes, develop individualized care plans, and coordinate services. The role emphasizes skilled nursing care for patients recovering from illness or managing chronic conditions, with collaboration across physicians, therapists, and the care team.

Qualifications

  • Current, unrestricted RN license.
  • Graduate of an accredited nursing program.
  • Minimum one year of nursing experience; home health experience preferred.
  • Knowledge of Medicare, home health regulations, and clinical documentation standards.
  • Strong assessment, clinical decision-making, and organizational skills.
  • Excellent communication and patient education abilities.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized plans of care.
  • Provide skilled nursing services, including wound care, medication management, IV therapy, and disease management education.
  • Monitor patient progress and adjust care plans as needed.
  • Educate patients and family members on treatment plans, medications, and disease processes.
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Identify changes in patient condition and communicate appropriately with the healthcare team.
  • Participate in case conferences and interdisciplinary team meetings.
  • Ensure compliance with agency policies, state regulations, and Medicare guidelines.
