Job summary

AdvantageCare Rehabilitation is seeking a PT for weekend PRN coverage at Holy Redeemer Lafayette – Fox Chase in Philadelphia, PA. You will work with adult and geriatric residents in a skilled nursing and rehabilitation environment, evaluating mobility and functional goals, and delivering active therapy.

The role emphasizes weekend availability, independent practice with clinical autonomy, and collaboration with an interdisciplinary rehab team to maintain high-quality patient care.

Qualifications

• DPT required; PA license or eligibility to practice in PA.
• Experience evaluating and treating adult/geriatric populations in SNF/post-acute settings preferred.
• Strong documentation and communication skills are essential.
• Ability to work independently and as part of a rehab team.
• Weekend PRN availability with reliable scheduling is required.

Responsibilities

• Conduct comprehensive PT evaluations and develop individualized treatment plans.
• Provide skilled PT interventions focusing on strength, balance, mobility, endurance, and independence.
• Assess and treat gait abnormalities, fall risk, and mobility limitations.
• Implement exercises, neuromuscular re-education, and functional training.
• Support residents recovering from surgery, illness, or hospitalization; educate patients, families, and caregivers.
• Collaborate with OT, SLP, nursing, and other team members; complete progress notes and discharge recommendations.
