AdvantageCare Rehabilitation is seeking a Weekend PRN Physical Therapist (PT) for Holy Redeemer Lafayette – Fox Chase in Philadelphia, PA. The role focuses on weekend coverage in a skilled nursing and geriatric rehabilitation setting to support adult and elderly residents.

You will perform evaluations, develop treatment plans, and implement interventions to improve mobility, balance, and independence. The position offers flexible weekend hours and opportunities to stay clinically active

Formación

• Doctor of Physical Therapy (DPT) or equivalent degree required.
• Current Pennsylvania Physical Therapist license or eligibility required.
• Strong clinical evaluation, treatment planning, and documentation skills.
• Ability to provide skilled physical therapy services independently within professional standards.
• Experience with adult or geriatric populations preferred.
• Skilled nursing, post-acute or long-term care experience is a plus.
• Strong communication, time management, and interdisciplinary collaboration skills.
• Availability to provide PRN weekend coverage.
• A compassionate, patient-first approach to rehabilitation.

Responsabilidades

• Conducting comprehensive physical therapy evaluations and developing individualized treatment plans
• Providing skilled PT interventions focused on strength, balance, mobility, endurance, and functional independence
• Assessing and treating gait abnormalities, fall risks, and functional mobility limitations
• Implementing therapeutic exercises, neuromuscular re-education, and functional training
• Supporting residents recovering from surgery, illness, injury, or hospitalization
• Providing patient, family, and caregiver education on mobility, safety, and home exercise programs
• Collaborating with OT, SLP, nursing, and other members of the interdisciplinary team
• Completing timely and accurate evaluations, treatment documentation, progress notes, and discharge recommendations
