AdvantageCare Rehabilitation in Philadelphia is seeking a Weekend PRN Physical Therapist (PT) to support Holy Redeemer Lafayette – Fox Chase in a skilled nursing & rehabilitation setting. You will evaluate, treat, and document care for adult and geriatric patients on a weekend schedule.

The role offers flexible PRN weekend coverage, opportunities to stay clinically active, and a supportive interdisciplinary team that values clinical autonomy and quality patient care.

Qualifikationen

• DPT from an accredited physical therapy program.
• PA license or eligibility prior to employment.
• Strong evaluation, treatment planning and documentation skills.
• Ability to provide skilled PT services independently in SNF/post-acute settings.
• Experience with adult/geriatric populations preferred.
• SNF/post-acute care experience is a plus.

Aufgaben

• Conduct comprehensive PT evaluations and develop individualized treatment plans.
• Provide skilled PT interventions focusing on mobility, balance and functional independence.
• Assess gait, fall risk and functional mobility; adjust plans as needed.
• Document evaluations, progress notes and discharge recommendations.
• Collaborate with OT, SLP, nursing and other team members.
