Acts Retirement-Life Communities, Inc. is seeking a Physical Therapist to provide PT services across the continuum of care for residents, including evaluations, treatment planning, actual treatments, and discharge planning.

Qualifications

• Bachelor’s degree in Physical Therapy.
• Valid PT license or eligibility for license in the state(s) where services are rendered.
• Experience in a skilled nursing and/or outpatient rehab environment (preferred).

Responsibilities

• Provide physical therapy services to residents, including evaluations, planning individual treatment programs and goals, actual treatments, and discharge planning.
• Perform other related duties as required.
