Job summary

Acts Retirement-Life Communities is seeking a skilled Physical Therapist to join our in-house rehab program in a welcoming, slower-paced setting. You will evaluate residents, plan individualized treatment programs, deliver therapies, and participate in discharge planning while coordinating with inpatient, home health, and outpatient services.

The ideal candidate has a Physical Therapy license and experience in SNF/outpatient rehab.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• Valid Physical Therapist license or eligibility in the state(s) of practice.
• Experience in skilled nursing and/or outpatient rehab is preferred.

Responsibilities

• Provide physical therapy services to residents including evaluations and planning individualized treatment programs.
• Administer treatments, discharge planning, and related duties; follow residents across care continuum as needed.
