Job summary

Generations Healthcare is seeking a Certified Licensed Occupational Therapy Assistant to join our skilled nursing and rehabilitation facility on a per diem, on‑call, weekend basis. You will support licensed therapists to deliver high‑quality patient care and help residents achieve their goals.

Responsibilities include implementing treatment plans, documenting progress, and collaborating with families and the care team.

Qualifications

• Certified Licensed Occupational Therapy Assistant required.
• Must be in good standing with regulatory agencies.
• Knowledge of Medicare and payer sources is preferred.

Responsibilities

• Implements and executes on established patient treatment plans under supervision.
• Communicates with patients, families, physicians and team members.
• Provides timely written documentation per facility requirements.
