Job summary

Harbor Hospice of Lake Charles is seeking a Weekend On-Call RN Case Manager to join our Lake Charles area team. The role focuses on admissions, discharges and pronouncements, with support for the Triage Nurse.

Qualified candidates will hold a Louisiana RN license and have reliable transportation for travel. Hospice or Home Health experience is preferred, along with strong communication and a clean background.

Qualifications

• Louisiana RN license required.
• 1+ year RN experience preferred.
• Hospice or Home Health experience preferred.
• Strong written and verbal communication.
• The ability to travel using reliable transportation.
• Clean background, drug screen, and driving record.

Responsibilities

• Admissions, discharges and pronouncements.
• Assist the Triage Nurse and perform select assessments.
