Job summary

Pathwell Health in Connecticut seeks a Physical Therapist to cover on-call shifts from Friday 4:30 pm to Monday 8:00 am, plus as-needed and weekend visits. Compensation includes an on-call stipend plus per-visit pay.

The PT assesses health problems, develops care plans, and maintains medical records, serving Greenwich, Stratford, New Haven and surrounding areas. Licensing in Connecticut is required; bilingual Spanish is a plus.

Qualifications

Graduation from an approved Physical Therapy curriculum.
Current Connecticut Physical Therapist license.
Experience in home health is preferred.
Bilingual Spanish preferred.

Responsibilities

Develops and adjusts a conditioning/rehabilitation plan of care with client and physician.
Tests and screens physical strength to aid evaluation of function.
Educates client and family about exercises and therapeutic procedures.
Documents observations and progress in patient records.
Provide direct patient care as prescribed and participate in treatment planning.
Review referrals and medical records to determine required PT treatment.
Record prognosis, treatment and progress in charts.
Identify goals and plans for reevaluation.
Evaluate treatment effects and adjust as needed.
Participate in educational activities and clinical studies.
Perform other duties as assigned.
