Job summary

Pomelo Care, the leading virtual medical practice for women and children, seeks an experienced Registered Nurse with a BSN to provide patient-centered care across pregnancy and the postpartum period. The role involves remote assessments, care planning, and coordination with a multidisciplinary team from anywhere in the continental US.

You will support patients via video, phone, and asynchronous chat, while traveling occasionally and working weekends/holidays as needed.

Qualifications

• RN with minimum BSN and active compact RN license with multi-state privileges.
• 4+ years in hospital/healthcare, with at least 3 years in Obstetrics.
• Willing to travel occasionally; weekends and holidays.
• Aware of birth inequity and structural racism in maternal health.
• Professional engagement and ongoing learning.

Responsibilities

• Ongoing clinical and psychosocial assessment of new patients and building rapport.
• Establishing individualized care plans based on evidence-based standards.
• Developing wellness goals and motivating factors for outcomes.
• Identifying and addressing barriers to care identified by patients or care team.
• Reducing care gaps through frequent patient engagements.
• Educating patients on remote monitoring equipment and its use.
• Timely review of abnormal diagnostic results and triaging as needed.
• Escalating to multidisciplinary teams when appropriate.
• Documenting all care/interactions promptly.
