Qualifications

• Active RN or LPN licensure in the state of practice.
• 1+ year of nursing experience required.
• Experience in Long-Term Care, rehab/SNF, or sub-acute care preferred or nursing leadership experience.

Responsibilities

• Supervises care planning and delivery to ensure quality patient care.
• Assess residents on admission and as conditions change.
• Performs rounds to evaluate patient condition.
• Provides or assists with direct patient care as needed, including ADLs, meals, medications, and treatments.
• Obtains and implements Physician Orders, ensuring accuracy.
• Communicates between shifts about changes in resident conditions.
• Ensures a safe environment for patients, staff and visitors.
• Provides clinical evaluations to MD/NPs on call for quality outcomes.
• Plans and evaluates staff competencies and ongoing education.
• Participates in facility surveys and discharge planning updates.
• Ensures adequate nursing staffing per shift.
• Performs daily rounds and reports findings to the Director of Nursing Services.
