Job summary

Aviata Health Group is seeking a Nurse Practitioner to lead internal medicine services at a Skilled Nursing Facility on Saturdays and Sundays. You will work with the facility’s team and medical directors to deliver high-quality care for short- and long-term residents.

The role requires an active Florida NP license, a DEA number, and a Master’s degree. You will perform comprehensive assessments, manage chronic conditions, and coordinate discharge and admissions in a fast-paced setting.

Qualifications

• Must hold Advanced Practice Licensure (APN) with Florida license.
• Active license and DEA number required.
• Master’s degree in Nurse Practitioner.
• Experience in nursing or other medical fields.
• Excellent communication and leadership in healthcare.

Responsibilities

• Provide primary internal medicine on Saturdays and Sundays in SNF.
• Collaborate with interdisciplinary team and physicians to reduce weekend hospitalizations.
• Evaluate new admissions and coordinate discharge visits.
• Perform comprehensive physical examinations and prescribe treatments under protocols.
• Develop patient management policies and facilitate referrals.
