Qualifications

• Bachelor's Degree in Nursing or Social Work; RN license preferred or LVN/LPN acceptable.
• Two years clinical nursing experience preferred; knowledge of Medicare/Medicaid beneficial.
• Ability to read and interpret policy manuals and procedural documents.
• Strong documentation and communication with residents, families, and care team.

Responsibilities

• Communicate care plans and treatment protocols with residents and families.
• Attend stand-up meetings to facilitate team communication.
• Monitor and adjust resident statuses; organize daily work for balance.
• Oversee admissions and discharges; coordinate referrals.
• Accurate charting and timely paperwork; maintain professional relationships.
