RHA Health Services, LLC is hiring an on-site Licensed Practical Nurse (LPN) in Trenton, TN. This full-time role covers every other weekend from 8a to 8p, supporting individuals with intellectual and developmental disabilities, as well as mental health and substance use challenges.

You will provide holistic nursing care, assist the RN, administer medications, train unlicensed staff, and coordinate with physicians and the Interdisciplinary Team to promote health.

Qualifications

• Graduation from an accredited licensed nurse program.
• Current nursing license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to obtain certification.
• Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.
• Must be able to work independently, be organized, and possess the ability to prioritize work tasks.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities
• Assist the Registered Nurse in delivering quality nursing services to persons supported
• Provide health related training to unlicensed personnel
• Administer medications and treatments or assist the person supported with medication administration
• Assist with ensuring preventative and routine medical appointments are scheduled
• Conduct medication closest assessments
• Collect health related data
• Observe for and report any abnormal health related findings to the physician
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
• Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported
