Job summary

Vitality Hospice & Palliative Care in New Jersey is seeking a Per Diem Licensed Practical Nurse (LPN) to deliver compassionate, patient‑centered hospice care in patients' homes or facilities on weekends. The role offers flexibility, autonomy, and meaningful one‑on‑one care, with local travel and mileage reimbursement.

Under RN supervision, you will perform assessments, manage pain and symptoms, assist with ADLs, educate families, and document visits.

Qualifications

• Active LPN license in the state of practice.
• 1 year nursing experience preferred, hospice experience a plus.
• Knowledge of hospice philosophy and comfort care principles.
• Strong assessment, communication, and documentation skills.

Responsibilities

• Provide skilled hospice nursing care per patient care plan.
• Perform nursing assessments and report changes promptly.
• Support pain and symptom management for comfort.
• Assist patients with ADLs as needed.
• Educate patients, families, and caregivers on care needs.
• Accurately document visits and care provided.
• Collaborate with interdisciplinary hospice team for coordinated care.
• Participate in on-call rotation as available (per diem).
