Job summary

Brickyard Healthcare is seeking a Weekend Licensed Practical Nurse (LPN) in Bloomington, MN to provide compassionate, quality nursing care and support to residents. The role emphasizes team collaboration, discharge planning, and implementing individualized care plans to achieve positive outcomes.

Qualifications

• Must hold and maintain a current license to practice as an LPN.
• Must hold and maintain a current CPR certification; CPR within 90 days of hire.
• License in good standing with the state; no findings of abuse, neglect, or misappropriation.

Responsibilities

• Ensure quality nursing care using the nursing process and achieve positive resident outcomes.
• Identify needs and provide input for discharge planning and follow-up.
• Develop individualized care plans with the interdisciplinary care team.
• Implement care plans consistently, efficiently, and with focus on resident outcomes.
• Collaborate with physicians and other health care professionals by sharing information relevant to changes in plan of care.
• Deliver and maintain resident comfort and care by applying current nursing practice.

Baylor Schedule
Work three 12-hour shifts every weekend (Friday, Saturday, and Sunday) and receive Baylor incentive pay in accordance with company policy.

Opportunities are available to pick up additional shifts and earn incentive bonuses.

Responsibilities:

• Assure that effective quality nursing care is delivered which is outcome focused through utilization of the nursing process.
• Identify needs and provide input for discharge planning and follow-up throughout length of stay
• Assist with development of individualized plan of care in collaboration with the resident/responsible party and interdisciplinary care team
• Implement plan of care consistently, effectively and cost efficiently with focus on resident centered outcomes by utilizing all appropriate, available resources for resident, family or responsible party counseling and discharge planning.
• Work in collaboration with physician and other health care professionals by sharing information relevant to changing plan of care
• Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practice

Qualifications:

• Must hold and maintain a current license to practice as a Licensed Practical Nurse (LPN)
• Must hold and maintain a current CPR certification. Licensed nursing staff are required to obtain their CPR certification within 90 days of hire.
• License in good standing with the state. Must have no finding in the registry concerning abuse, neglect, or misappropriation of resident property.
