Qualifications

• MSW from an accredited school of social work.
• Registered as an Associate Clinical Social Worker with the Arizona Board of Behavioral Health Examiners.
• Prior experience in a healthcare setting.
• Familiarity with regulatory requirements related to social work and healthcare.
• Strong communication skills (verbal, non-verbal, written, and oral).

Responsibilities

• Conduct comprehensive psychosocial assessments of patients and families.
• Develop, implement, and regularly update individualized care and discharge plans.
• Provide counseling, crisis intervention, and supportive services to patients and families.
• Collaborate with the interdisciplinary healthcare team to coordinate patient care.
• Educate patients and families on available community resources and support services.
• Make mandated reports and ensure compliance with all legal and regulatory requirements.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in team meetings, case conferences, and quality improvement activities.
