Job summary

Responsibilities include basic nursing care per Plan of Care, transfers, hygiene, meal prep, companionship, light housekeeping, and accurate record-keeping. DFS certification and a high school diploma or GED are preferred; strong communication and dependable teamwork are essential.

Qualifications

• Maintain certification with DFS.

• Possess effective communication skills to read, write and follow directions.
• Ability to perform physical tasks necessary to treat clients who may include lifting, bending and standing.

Responsibilities

• Provide direct patient care as assigned.
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises as instructed by supervising professionals.
• Prepare meals with consideration of client dietary needs.
• Provide companionship and light housekeeping.
• Maintain complete and accurate records required by agency.
• Record and report changes in client's condition.
• Follow Plan of Care and uphold patient confidentiality.
