Qualifications

• Registered Nursing license issued by the State of Illinois.
• Bachelor's of Science Nursing (BSN) degree.
• BLS certification from American Heart Association.

Responsibilities

• Performs a comprehensive assessment on all assigned patients with documented findings.
• Communicates changes in patient condition promptly to the care team.
• Uses nursing diagnoses and outcomes to update the care plan and goals.
• Identifies and prioritizes patient needs and documents them accordingly.
• Collaborates with patient, family and health team for discharge planning and referrals.
• Monitors patient response to interventions and adjusts the plan as needed.
• Ensures discharge instructions and medication reconciliation are provided.
