Job summary

Qualifications

• Active Ohio RN license required.
• Current CPR certification.
• CHPN certification required or must be obtained within two years of employment.
• Valid driver’s license with safe driving record.
• State-required auto insurance coverage.

Responsibilities

• Receive and triage patient and family calls based on clinical urgency and needs.
• Provide compassionate clinical guidance and customer service.
• Coordinate services according to the established plan of care.
• Communicate changes in patient condition to the clinical staff.
• Document assessments, interventions, and communications in the EMR.
• Support continuity of care to promote comfort and quality of life.
