Job summary

Pathways Healthcare is seeking a weekend in-home nurse to join our physician- and nurse practitioner-led team in Westwood, MA. This part-time role covers Saturdays and Sundays from 8:30 am to 8:00 pm, providing preventive and rehabilitative nursing care and following the plan of care in patients' homes.

You will administer medications, coordinate with physicians and the interdisciplinary team, respond to after-hours calls, and ensure high-quality patient care with adherence to regulations.

Qualifications

• 2+ years nursing experience, preferably geriatrics.
• Nursing degree from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and revises as patient needs change.
• Responds to afterhours calls on weekends (8:30 AM–8:00 PM).
• Assesses patient needs through triage and in-home care.
• Collaborates with physicians and interdisciplinary team on care plans.
• Responds to inquiries from patients and caregivers.
• Consults patients and caregivers before, during and after treatment.

Position Details:

• Main Office location: Westwood, MA
• Service Area: Westwood, Quincy, Weymouth, Holbrook, Brockton, Attleboro, Taunton areas
• Job Type: Part-time
• Schedule: Saturday and Sunday 8:30 am – 8:00 pm

Responsibilities:

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.

Qualifications:

Benefits & Compensation:

• Pay Structure: Base Compensation ($600 per weekend) + Paid per visit ($50) and per admission ($100). Visits over 60 minutes are paid at $75. Training and meetings are paid hourly at $35/hr.
