Qualifications

• Associate Degree in Nursing (required).
• PA RN license (required).
• 2+ years hospice experience preferred.

Responsibilities

• Provide management and delivery of individualized hospice patient care plans.
• Support patients in Gap, PA and surrounding areas.
• Conduct comprehensive assessments and ongoing evaluations of patient needs.
• Consistently meet critical deadlines for charting and documentation.
• Collaborate with interdisciplinary hospice team members to ensure coordinated care.
• Educate patients and families on care plans, symptom management, and available resources.
• Self-manage time, schedule, and daily patient visits.
• Maintain compliance with regulatory, clinical, and organizational standards.
