Job summary

Qualifications

• Current RN license in the practicing state.
• At least two years of clinical nursing experience.
• Hospice nursing experience preferred.
• CPR certification.
• Willingness to travel and weekend/holiday call.

Responsibilities

• Provide care and document care in accordance with reimbursement guidelines.
• Coordinate physical care of the patient with family, patient, physician and Hospice team.
• Communicate with patients and families at all levels.
• Communicate with all agency personnel.
• Attend IDT meetings and quality reviews as scheduled.
