Qualifications

• Graduate of an accredited nursing program.
• Current licensed Registered Nurse (RN) in the state of practice.
• One year of Hospice experience within the last 24 months.
• Valid driver’s license and reliable transportation.
• Proof of automobile insurance (MVR checked).
• Proof of current CPR certification.
• Able to pass criminal background check and national sex offender clearance.

Responsibilities

• Provide hospice nursing care under the plan of care and coordinate with the interdisciplinary team.
• Manage the patient’s care from admission through discharge and coordinate bereavement involvement.
• Educate the patient and family on their rights and responsibilities as a Hospice patient.
• Communicate with the Attending Physician and Hospice Medical Director to keep them informed of changes in condition.
• Initiate patient care conferences for complex patients and ensure continuity of care across settings.
