Job summary

Alpha Home Health & Hospice in Snohomish County is seeking an experienced Part-time Weekend Registered Nurse (RN) Case Manager for our Hospice division. You will provide skilled nursing care, develop individualized care plans, and educate patients and families in end-of-life care.

Qualifications

• Active Washington State RN license is required.
• Prior home health and/or hospice experience preferred.
• Current BLS certification from the American Heart Association.
• Valid driver’s license with insured vehicle.

Responsibilities

• Provide skilled nursing care and symptom management for terminally ill patients.
• Develop and manage individualized care plans with the hospice team.
• Educate and support patients and families through end‑of‑life care.
• Administer medications and treatments for comfort and quality of life.
• Coordinate care with physicians, social workers, aides, and other team members.
• Respond to urgent needs and provide crisis support when necessary.
• Maintain accurate and timely documentation in the EMR.
• Ensure compliance with hospice regulations and standards.
