Job summary

Care Initiatives Hospice - Greenfield is seeking a dedicated Hospice Nurse (RN) to provide comprehensive, compassionate care across a variety of settings including home, long-term care, assisted living, and hospital.

As part of the on-call weekend package, you will cover weekends from Friday 4:30 PM to Monday 8:00 AM and may be assigned additional visits, supporting patients and families with comfort, symptom relief, and end-of-life care.

Qualifications

• RN with a minimum of two years of recent clinical nursing experience.
• Current CPR certification.
• Demonstrated patience and a supportive approach to patient, family, and caregiver support.
• Willingness to provide care in home and facility settings.

Responsibilities

• Provide weekend/on-call coverage with Fridays 4:30 PM to Mondays 8:00 AM as part of the full-time package.
• Support patients and families in times of greatest need, addressing emergent needs during weekends and evenings.
• Administer meds as available and provide teaching to ensure comfort, pain management, and dignity in all interactions.

What you’d do as the Hospice Nurse - RN - On Call Weekend Package:

• This is a full-time role that is takes call from Fridays at 4:30PM until 8:00AM on Monday. Some scheduled visits may also be assigned.
• Support patients and families in the time of their greatest need and respond to emergent needs on weekends, evenings and overnight.
• Assist the caregivers and patients, provide meds available and offer teaching to ensure comfort for pain, breathlessness, dying/transitioning or support at the time of death with compassion, integrity, respect, and dignity in all our interactions.
