By The Bay Health is seeking an After-Hours Nurse to provide skilled nursing and hospice care, maximizing comfort and health for patients in a per diem role. Schedule: Every Friday, Saturday & Sunday, 8:30am–5:00pm, serving San Francisco and Marin counties.

CA RN license required; hospice experience preferred; CDPH vaccine compliance. Qualifications include one year of nursing experience in the past three years or a BSN, and a CA license; Public Health Nursing or HPNA certification is desirable.

Qualifications

• Current California RN license.
• One year nursing experience in the past three years or a Bachelor’s degree in Nursing.
• Experience in acute care/home health and/or hospice setting preferred.
• Public Health Nursing certification desirable; HPNA certification preferred.

Responsibilities

• Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
• Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
• Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family.
• Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.
