Job summary

Anchor Health is seeking a per diem After-Hours Nurse to deliver skilled nursing and hospice care, enhancing comfort and health for patients in San Mateo and Santa Clara Counties. The role operates on Saturdays and Sundays, under the After-Hours Nurse's guidance, with a focus on coordinated, compassionate care.

Candidates should have at least one year of professional nursing experience (or a Bachelor’s in Nursing) and preferably experience in acute care, home health, or hospice settings.

Qualifications

• One year of professional nursing experience in the past three years or a Bachelor's in Nursing.
• Experience in acute care/home health and/or hospice preferred.

Responsibilities

• Provide continuing care to patients and families in accordance with agency policies and regulations.
• Under supervision of the After-Hours Nurse, assess, plan, implement, and evaluate nursing services per the care plan.
• Coordinate with attending physicians and community resources to ensure optimal patient care and resource use.
• Maintain regular communication with Team Leaders about patient/family status and issues.

Coverage Area:

San Mateo and Santa Clara County

Essential Duties & Responsibilities include (but are not limited to):

• Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
• Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
• Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family.
• Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.

Education/Experience:

• One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

License:

• Current license as a registered professional nurse in the State of California orLicensed Vocational Nurse in the State of California
