Job summary

Constellation Health Services is seeking a Saturday/Sunday Weekend GIP Registered Nurse with a focus on hospice care. This role covers patients in the Greenwich, Stamford, and Norwalk area, with eight- or ten-hour shifts on Saturdays and Sundays.

Weekend coverage and liaison duties with the care team are highly valued. Graduate nurses with Hospice care interest and RN licensure in CT are encouraged to apply.

Qualifications

• Graduate of an accredited RN program.
• RN licensed in the state of employment.
• Hospice care experience preferred.
• Strong communication and teamwork with interdisciplinary teams.

Responsibilities

• Provide skilled nursing care and administer medications per physician’s plan.
• Perform comprehensive patient assessments and coordinate care with care team and family.
• Document thoroughly using the agency’s electronic system in a timely manner.
• Participate in weekly Interdisciplinary Team Meetings and report changes in patient condition.
