Job summary

Suncoast Hospice of Hillsborough, a member of Empath Health, is seeking an Admissions RN to join our team in Pinellas County with the ability to travel throughout the service area, covering hospitals and field settings (homes, ALF, LTC).

Schedule: Friday - Sunday 10a-10p. The Admissions RN will assess eligibility and complete admissions per program guidelines while delivering compassionate, patient-centered care across care settings.

Qualifications

• Valid Florida RN license.
• Minimum of six months to one year related experience and/or training.
• Associate’s degree or equivalent from a two-year college or technical school.

Responsibilities

• Assess eligibility for Empath Health Network of Care and advocate for optimal access to the right care in any care setting.
• Complete the admission process based on care program specifics, guidelines, and patient needs.
• Demonstrate compassionate customer service and relationship-building with internal and external stakeholders.
