Job summary

Suncoast Hospice, part of Empath Health, seeks an Admissions RN to support patients in Pinellas County, with a Sunday–Wednesday 10:00 a.m. to 8:00 p.m. schedule. The role focuses on coordinating admissions across long-term care facilities, homes, hospitals, and hospice houses to ensure seamless transitions.

You will educate patients and families about the terminal diagnosis, assess needs, and collaborate with a compassionate team to deliver Full Life Care.

Qualifications

• Must possess a clear and current Florida Licensure as RN.
• BSN or MSN preferred.
• Minimum one year experience in an acute-care Hospital/In-patient environment; hospice RN preferred.
• Ability to educate patients and families about disease processes and care needs.

Responsibilities

• Coordinates patient admissions activities across long-term care facilities, patient homes, hospitals, assisted living facilities and hospice houses.
• Educates patients, families and caregivers regarding disease processes, palliative interventions, care giving, dying process and safety practices.
• Provides initial assessment of impact of terminal diagnosis on physical, functional, psychosocial and environmental needs and activities of daily living.
