Phoenix Home Care and Hospice in Springfield, MO is seeking an experienced Hospice Admissions RN for a weekend-shift, full-time role. You will lead admissions, coordinate with the interdisciplinary team, and provide compassionate, patient- and family-centered care as patients transition from curative to comfort-focused treatment.

The position emphasizes strong clinical judgment, communication, and the ability to work independently within a supportive hospice team.

Qualifications

• Hospice experience required.
• Active Missouri RN license.
• Valid driver’s license.
• Strong assessment, organization, and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Complete hospice admissions and skilled nursing visits.
• Provide compassionate care and support to patients and families.
• Educate patients and families on conditions such as CHF, COPD, and diabetes.
• Provide wound care and medication management.
• Document patient conditions and communicate changes to physicians.
• Collaborate with the interdisciplinary hospice team.
