Job summary

Phoenix Hospice in Springfield, MO is seeking a Weekend Hospice Admissions RN to join our compassionate team. Full-time, 30-40 hours per week across 10-hour shifts Saturday through Monday; you will guide patients and families as they transition from curative to comfort-focused care with clinical excellence and dignity.

You’ll collaborate with intake coordinators, medical directors, facility staff, and physicians to ensure admissions are smooth and centered on what matters most.

Qualifications

• Hospice experience required.
• Active Missouri or Compact RN license.
• Valid driver's license.
• Strong assessment, organization, and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Complete hospice admissions and skilled nursing visits.
• Provide compassionate care and support to patients and families.
• Educate patients and families on conditions such as CHF, COPD, and diabetes.
• Provide wound care and medication management.
• Document patient conditions and communicate changes to physicians.
• Collaborate with the interdisciplinary hospice team.
