Job summary

Phoenix Hospice in Springfield, MO & surrounding areas is seeking a Weekend Hospice Admissions RN. Full Time: 30-40hrs, 10-hour shifts, Saturday to Monday. You will guide patients from curative to comfort-focused care with compassion and clinical excellence.

You’ll collaborate with intake coordinators, hospice consultants, medical directors, facility staff, and physicians to ensure admissions are smooth, thoughtful, and centered on what matters most.

Qualifications

• Hospice experience required.
• Active Missouri or Compact RN license.
• Valid driver’s license.
• Strong assessment, organization, and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Complete hospice admissions and skilled nursing visits.
• Provide compassionate care and support to patients and families.
• Educate patients and families on conditions such as CHF, COPD, and diabetes.
• Provide wound care and medication management.
• Document patient conditions and communicate changes to physicians.
• Collaborate with the interdisciplinary hospice team.
