Job summary

Lehigh Valley Health Network (LVHN) is seeking a Registered Nurse (RN) Hospice Admissions Nurse for weekend daytime shifts in Allentown, PA. This role determines hospice needs, performs initial and follow-up assessments, and develops patient-centered care plans in collaboration with physicians and the interdisciplinary team.

Qualifications

• Bachelor’s degree in nursing or ADN with 1 year hospice/home health experience.
• Two years RN experience in acute care or hospice/home health.
• BLS certification required; ability to travel within service area.

Responsibilities

• Perform initial and follow-up hospice assessments.
• Develop individualized care plans per physician orders.
• Collaborate with the hospice interdisciplinary team.
• Educate patients and families on end-of-life care and symptom management.
