Job summary

Personal Home Care of North Carolina is hiring Home Health Registered Nurses (RN) for the Charlotte area. PRN, part-time, or full-time positions with a sign-on bonus up to $10,000 are available. RNs provide in-home nursing care under physician direction, coordinating with patients, families, and care teams.

Responsibilities include assessments, treatments, education, documentation, and participation in weekly MDT meetings. NC or multi-state RN license and NC driver's license required.

Qualifications

• Prior Home Health experience is preferred.
• OASIS knowledge is a plus.
• Excellent time management, organizational, and reporting skills.
• Strong communication skills, both written and verbal.
• Great technology skills.
• Current NC or multi-state RN license.
• Current driver's license and auto liability insurance issued in NC.

Responsibilities

• Provision of nursing care, including assessments, treatments, diagnostics, and patient education per plan of care and orders.
• Observe signs, drug responses, and patient changes; report to supervisor/physician as needed.
• Coordinate care with patient/responsible party, physicians, other disciplines, and resources.
• Document accurately and timely per regulations and agency policy.
• Participate in multi-disciplinary team meetings weekly.
• Engage in learning and job-enhancement activities as directed by supervisor.
• Participate in On-Call rotation; one weekday on-call required.
