Job summary

Elite Home Health, part of LHC Group and a member of the Optum family, seeks a Weekend Registered Nurse in Home Health to provide and direct nursing care to patients in their homes per physician orders and agency policies.

You will coordinate the total plan of care with the health team, monitor care delivery, ensure accurate documentation, and participate in on-call rotations to support continuity of care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to reliable transportation.

Responsibilities

• Initiates, develops, and revises the plan of care with physicians and other professionals.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks.
• Performs comprehensive assessments, medication reconciliation, and timely documentation.
• Communicates with physicians, patient families, and care teams to ensure quality outcomes.
