Qualifications

• Current NV nursing license in good standing.
• Experience in Medicare Certified Home Health with OASIS Start of Care documentation.

Responsibilities

• Conduct comprehensive head-to-toe assessments and monitor changes.
• Provide acute and post-acute home health care, including wound care and IV therapy.
• Triage and manage urgent clinical situations in home settings.
• Perform medication reconciliation, patient teaching, and safety monitoring.
• Document wound assessments and care thoroughly.
