Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team in Arizona, with weekend responsibilities and visits focused on Start of Care/Admissions. The role requires Home Health and OASIS documentation experience and the ability to travel across the coverage area.

Per Diem, Part Time, and Full Time schedules are available, offering competitive pay and flexible scheduling to support independent practice in patients' homes.

Qualifications

• AZ nursing license in good standing.
• Graduation from an accredited nursing program.
• At least one year of clinical experience with adult/geriatric populations in acute/hospital, home health, hospice, or rehab settings.

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition.
• Apply knowledge of acute and post-acute care (CHF, COPD, wound care, IV therapy).
• Triage and manage urgent clinical situations in the home setting.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.
