Job summary

Enhabit Home Health & Hospice in Oregon is seeking a Full-time Weekend RN to join our field-based care team. Weekend coverage is required with Saturday and Sunday scheduling, delivering skilled nursing visits and coordinating patient care in home settings.

As the primary liaison across disciplines, you’ll ensure accurate assessment, care plans, and data collection where applicable, while collaborating with physicians and office staff to support optimal patient outcomes.

Qualifications

• Be currently licensed as a Registered Nurse (RN) in the state of employment.
• A minimum of one year of clinical experience is preferred.
• Demonstrate knowledge and skill in current nursing practice.

Responsibilities

• Performs skilled nursing visits and coordinates client care.
• Point of contact for all disciplines involved with providing care to patients.
• Oversees the frequency of visits for the episode.
• Consults with physician and office with patient care details.
• Performs accurate OASIS collection where applicable and ensures up-to-date medication profiles.
