Job summary
Holy Cross Home Care and Hospice (Maryland) is seeking a Weekend Home Health RN to deliver skilled nursing in patients' homes, creating individualized care plans and educating families. You’ll work autonomously with a strong focus on 1:1 patient relationships, utilize Epic EHR for documentation, and enjoy a balanced schedule with no on-call requirements and comprehensive Day 1 benefits.
Minimum of one year nursing experience and Maryland RN license are required, BSN preferred.
Qualifications
- Minimum of one year of professional nursing experience.
- Recent home health experience or at least three years of acute care in Medical-Surgical, ICU, Emergency Department or Rehabilitation.
- Graduate of an approved Nursing education program; BSN preferred.
Responsibilities
- Deliver skilled nursing care in patients' homes.
- Create individualized care plans and administer medications.
- Educate patients and families and support recovery and independence.
- Utilize Epic EHR for documentation and care coordination.
- Provide 1:1 patient care with a focus on safety and quality outcomes.
