Job summary

Holy Cross Home Care and Hospice (Maryland) is seeking a Weekend Home Health RN to deliver skilled nursing in patients' homes, creating individualized care plans and educating families. You’ll work autonomously with a strong focus on 1:1 patient relationships, utilize Epic EHR for documentation, and enjoy a balanced schedule with no on-call requirements and comprehensive Day 1 benefits.

Minimum of one year nursing experience and Maryland RN license are required, BSN preferred.

Qualifications

• Minimum of one year of professional nursing experience.
• Recent home health experience or at least three years of acute care in Medical-Surgical, ICU, Emergency Department or Rehabilitation.
• Graduate of an approved Nursing education program; BSN preferred.

Responsibilities

• Deliver skilled nursing care in patients' homes.
• Create individualized care plans and administer medications.
• Educate patients and families and support recovery and independence.
• Utilize Epic EHR for documentation and care coordination.
• Provide 1:1 patient care with a focus on safety and quality outcomes.

Minimum Qualifications

• Must have a minimum of one (1) year professional nursing experience.

• Recent home care experience or at least three years of acute care experience in one or more of the following areas:

  • Medical-Surgical

  • ICU

  • Emergency Department

  • Rehabilitation

• Must have RN License within the State of Maryland RN License

• Graduate of an approved Nursing education program, BSN preferred.

• Strong computer and technology skills required.

• Must have current Driver’s license and reliable transportation.
