Concierge Home Care in Melbourne, FL is seeking a Weekend Home Health RN to deliver 1:1 patient care in a flexible, autonomy-driven role. The 4‑day schedule (Wed–Sat or Sun–Wed) offers higher earnings potential and a pay range of $80,000–$95,000+ annually based on experience and territory coverage across Melbourne/Brevard County.

The position emphasizes independence, scheduling control, and meaningful 1:1 care, with bonuses and benefits following completion of onboarding.

Qualifications

• Active, unencumbered Florida RN license.
• Valid driver’s license, reliable transportation, and auto insurance.
• Home health experience preferred, but not required.

Responsibilities

• Perform comprehensive patient assessments in the home setting.
• Develop and manage individualized care plans.
• Provide skilled nursing care and patient/caregiver education.
• Administer treatments and monitor patient condition.
• Coordinate care with physicians and interdisciplinary teams.
• Evaluate patient progress and adjust care as needed.
• Complete accurate, timely documentation using streamlined tools.
