Qualifications
- Active, unencumbered Florida RN license.
- Valid driver’s license and reliable transportation.
- Home health experience preferred, but not required.
Responsibilities
- Perform comprehensive home health assessments.
- Develop and manage individualized care plans.
- Provide skilled nursing care and education to patients and caregivers.
- Administer treatments and monitor patient condition.
- Coordinate care with physicians and interdisciplinary teams.
- Complete accurate documentation using streamlined tools.
