Job summary

Concierge Home Care is seeking a Weekend Home Health RN to deliver high-quality in-home nursing care in Port Orange, FL. This modified full-time role offers 4-day or weekend schedules, autonomy in patient care, and a competitive earning potential.

You will be responsible for 1:1 patient care with scheduling designed to minimize drive time and maximize caregiver-patient continuity, including a regional coverage area in Port Orange/Volusia County.

Qualifications

• Active Florida RN license.
• Valid driver's license, reliable transportation.
• Home health experience preferred, but not required.

Responsibilities

• Perform comprehensive patient assessments in the home setting.
• Develop and manage individualized care plans.
• Provide skilled nursing care and patient/caregiver education.
• Administer treatments and monitor patient condition.
• Coordinate care with physicians and interdisciplinary teams.
• Evaluate patient progress and adjust care as needed.
