Job summary

Interim HealthCare Inc. is seeking a Home Health Registered Nurse (RN) to provide skilled, patient-centered care in patients' homes across Greenville County, SC.

You will work closely with physicians and an interdisciplinary team to support safe recovery and independence, while maintaining high-quality outcomes. Qualifications

• Graduate of an accredited RN program.
• Active South Carolina RN license or Compact License.
• Minimum one year of nursing experience.
• Experience in Home Health, Med/Surg, Critical Care, or Case Management preferred.
• Familiarity with OASIS documentation preferred.

Responsibilities

• Provide skilled, patient-centered home care and coordinate with physicians and care team.
• Perform comprehensive patient assessments and accurate OASIS documentation.
• Develop, implement, and adjust individualized plans of care.
• Monitor patient condition and communicate changes to physicians to prevent complications.
• Educate patients and caregivers on medications, treatments, and home safety.
• Coordinate care with therapists, aides, and other providers for seamless service.
• Prepare patients and families for safe discharge and document care in the EMR.
