Interim HealthCare is seeking a Home Health Registered Nurse (RN) in Pickens, SC to deliver skilled, compassionate care at patients' homes. This role strengthens communities one patient at a time, helping manage chronic conditions and support independent living with coordinated care and thorough assessments.

You will work with physicians and the interdisciplinary team, perform Oasis documentation, educate families, and prepare for safe discharge.

Qualifikationen

• RN program graduate and licensed in SC.
• Experience in home health or related fields preferred.
• Familiarity with OASIS is a plus.

Aufgaben

• Provide skilled, patient-centered home health care.
• Perform comprehensive patient assessments and document accurately.
• Develop and adjust individualized care plans.
• Monitor patient status and communicate changes to physicians.
• Educate patients and families on medications, safety, and disease management.
• Coordinate with therapists and other providers for seamless care.
• Prepare patients for safe discharge and document in EMR.
